About the Role

As a Case Management Nurse, you will collaborate with the healthcare team to coordinate patient and family interventions across the continuum of care. Your role is to remove barriers and promote the efficient and effective use of resources while assisting patients and families in establishing specific goals, both long-term and short-term. You will facilitate appropriate follow-up care and refer patients to the appropriate next level or site of care, serving as an advocate to ensure safe healthcare.

In this position, you will act as a key information and education resource for the interdisciplinary team, demonstrating leadership in caring for complex needs and securing appropriate services. You will integrate ethical provisions into all areas of practice and contribute to developing and meeting key Pillar outcomes, including financial, satisfaction, and clinical quality of care goals.
