About the Role
As a Registered Nurse Case Manager (RNCM), you will bring comfort and dignity to our clients by supporting a full range of patient services. This role offers a career unlike any other, dedicated to work/life balance with truly flexible scheduling options including full-time, part-time, PRN, and weekends. You will provide 1:1 patient care with no overtime required and the option for daily pay.
In this position, you will conduct in-person patient interviews and comprehensive physical assessments. You will oversee the implementation and ongoing assessment of the patient's plan of care by managing home health aides, LPNs, RNs, and other caregivers. Your work involves communicating patient conditions and collaborating with appropriate providers to deliver care as needs evolve. Additionally, you will provide education to patients and families on proper home health care procedures, such as wound care, IV administration, and medication management, while working to decrease readmissions by promoting preventative care and ensuring continuity of care.

Requirements
