The Hospice Intake & Staffing Coordinator serves as a vital bridge between referral sources and the Home Health agency. This role is designed to support the efficient admission process and the strategic scheduling of clinical field staff. You will utilize your clinical expertise to determine the appropriateness of patient referrals for the home care setting, ensuring all actions align with professional scope of practice and Home Care regulatory standards.
In this position, you will coordinate the entire intake workflow, from verifying payer sources and processing orders to forwarding completed referrals to the Data Department. A key aspect of the role involves effective communication with clinicians, physicians, patients, caregivers, and referral sources. You will also participate in department activities aimed at improving care coordination between the Home Health Department, hospital discharge planners, and external partners.
