As a Triage Nurse, you will be a vital link in our network, monitoring and responding to patient communications while consulting with physicians and Advanced Practice Providers (APPs) when necessary. This role requires a deep recognition of the physical, psychological, and spiritual aspects of care. You will accurately identify patients with high-risk conditions and work within the RN scope of practice as defined by the Tennessee Board of Nursing and our internal policies.
Your day will involve addressing patient calls, portal messages, and emails concerning a wide range of symptoms and side effects. You will utilize approved algorithms to assess patient needs, document interventions, and aggressively manage symptoms to prevent unnecessary emergency department visits. You will collaborate with physicians, scheduling teams, and various departments to coordinate care, set appointments, and ensure effective communication. Additionally, you will provide clear education to patients and caregivers, counsel them on medication side effects and community resources, and act as an expert resource for the healthcare team and external agencies.
