Lee Health's Hospital at Home program brings hospital-level care directly to eligible patients in their own homes. This innovative service includes daily visits, remote monitoring, and specialized treatments, allowing patients to recover comfortably while reducing the risks associated with traditional hospital stays. The program reflects Lee Health's deep commitment to delivering patient-centered, cutting-edge care across Southwest Florida.
As a Registered Nurse in this PRN/Per Diem capacity, you will serve as a driving force in the Hospital at Home initiative. Your primary focus is visiting patients in their homes to deliver safe, high-quality nursing care. This role requires you to navigate the full nursing process—assessment, diagnosis, planning, implementation, and evaluation—using critical thinking and problem-solving skills to differentiate urgency in dynamic home environments.
You will act as a clinician, teacher, and leader, coordinating care based on established clinical standards from the American Nursing Association and the Florida Nurse Practice Act. Success in this role involves collaborating with other professional disciplines to ensure effective patient outcomes while honoring the organization's vision. You will apply your knowledge of patient age and cultural diversity to provide compassionate, ethical care that aligns with the code of ethics for nurses.
