About the Role
As a Registered Nurse (RN) in Home Health, you will serve as the central point of contact for all disciplines involved in providing care to patients. Your role extends beyond performing visits to overseeing the frequency of visits for each episode and coordinating client care. You will consult as needed with physicians and the office to provide detailed updates on patient care, ensuring accurate OASIS collection, maintaining current medication profiles, and timely reporting of lab values. Additionally, you will actively participate in weekly case conferences and monthly case manager meetings to drive collaborative care planning.
