About the Role
As a Registered Nurse (RN) in Home Health, you will serve as the central point of contact for all disciplines involved in providing care to patients. Your role involves overseeing the frequency of visits for each episode and coordinating client care. You will act as a key liaison, consulting with physicians and the office to provide detailed updates on patient care. The position requires accurate OASIS collection, maintaining current medication profiles, and ensuring lab values are reported to physicians in a timely manner. Additionally, you will participate in weekly case conferences and monthly case manager meetings to ensure comprehensive care delivery.
