About the Role

As a Registered Nurse in Home Health, you will be a vital part of our elite team, carrying out our mission every day by providing compassionate care in patients' preferred home settings. This role is designed for those with commitment and compassion who want to make a real difference in the lives of more than 60,000 patients.

In this position, you will have the autonomy to manage your own caseload, assessing and planning care for patients and families to identify their physical, psychosocial, and environmental needs. You will be responsible for the full continuum of care, from initial evaluations and re-evaluations to coordinating treatment plans and communicating significant findings to physicians and clinical supervisors. Your work ensures that every interaction aligns with the vision of our leadership and maintains the highest standards of privacy and confidentiality.
