About the Role
As a Registered Nurse Home Health, you will be a vital part of our mission to deliver unparalleled care. You will develop an amazing team of compassionate and dedicated healthcare providers while ensuring that all activities align with the vision of our board of directors and executive leadership.
In this position, you will assume primary responsibility for your caseload, assessing, planning, coordinating, implementing, and evaluating the plan of treatment. You will conduct initial evaluations and re-evaluations during each visit, communicating significant findings and changes in health conditions to the Clinical Supervisor, physicians, and other team members. Your work ensures that patients receive the right care, at the right time, in the right place.

Key Responsibilities
- check_circleAssess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs
- check_circleAssume primary responsibility for caseload including assessing, planning, coordinating, implementing, and evaluating treatment plans
- check_circleEvaluate patient needs and incorporate physical, psychological, social, and spiritual factors into the plan of care
- check_circleMaintain patient and staff privacy and confidentiality in compliance with HIPAA Privacy Final Rule
- check_circleRevise plans of care in response to identified patient care issues and notify relevant clinical staff and physicians
- check_circleCommunicate significant findings, health condition changes, and unsafe facility conditions to supervisors, physicians, and team members
- check_circleConduct initial and re-evaluations of patient nursing needs during each visit and schedule follow-up visits as needed
Requirements
- verifiedCurrent, unrestricted state RN licensee
- verified1+ year experience in a clinical care setting as a nurse
- verifiedValid driver's license
- verifiedDependable vehicle
- verifiedCurrent auto insurance
- verifiedAssociates Degree or Bachelor's Degree in Nursing (preferred)
- verifiedPrevious Home Health or Hospice Experience (preferred)
- verifiedPrevious experience with HomeCare HomeBase (preferred)
