About the Role
The Home Health Registered Nurse (RN) serves as a vital link in delivering high-quality, patient-centered care directly within the home setting. This position is designed for a clinician who thrives in an independent environment, utilizing strong clinical judgment to navigate complex patient needs while maintaining a collaborative spirit with an interdisciplinary team.
In this role, you will perform comprehensive patient assessments using OASIS documentation to develop and manage individualized care plans. Your day-to-day involves coordinating skilled nursing care in accordance with physician orders and agency guidelines, ensuring that every patient meets home health eligibility and medical necessity requirements. You will also play a key educational role, empowering patients and their families through instruction on disease management, medications, and care strategies.
Success in this position requires the ability to work autonomously in the field while effectively supervising and delegating tasks to LPNs and home health aides. You will be responsible for completing accurate and timely documentation in Homecare Homebase, ensuring continuity of care through seamless collaboration with physicians and therapists.
