About the Role
The Transitional Care Unit (TCU) is designed to ease patients into the life-changing journey of dialysis. The first weeks of treatment involve significant adjustment, carrying increased risks of hospitalization and mortality. As a TCU Registered Nurse, you will provide patients with a solid foundation to navigate this transition safely.
Your role goes beyond clinical care; you will coordinate education and support to help patients feel better and make informed decisions about their treatment modalities, including the choice between in-center or at-home dialysis. You will serve as a resource, leader, and mentor, guiding patients and their families through the complexities of End Stage Kidney Disease (ESKD) while collaborating with the interdisciplinary team to enhance patient outcomes.
