About the Role
This position is designed for a professional nurse dedicated to ensuring the overall coordination of home health services is delivered according to acceptable standards of practice and company procedures. You will play a pivotal role in reviewing and approving patient information submitted by licensed professionals during critical milestones such as start of care, recertification, resumption of care, or evaluation visits. Beyond clinical oversight, you will facilitate the essential relationship between physicians, referral sources, patients, caregivers, and employees, ensuring a seamless care experience.
