As a Registered Nurse Case Manager in Hospice, you will play a pivotal role in our mission to deliver high-quality care in patients' preferred home settings. This position is designed for a compassionate professional committed to assessing and supporting patients and their families through the physical, psychosocial, and spiritual aspects of their journey. You will be responsible for identifying patient needs, communicating significant findings to the clinical team and physicians, and ensuring that all care aligns with the vision of our leadership.
In this role, you will teach hospice philosophy and pain management to staff in alternate care settings, such as nursing homes and assisted living facilities. You will also provide essential support at the time of death and conduct bereavement assessments. Reporting to the Clinical Team Manager, you will help develop an amazing team of dedicated healthcare providers while ensuring compliance with the Interdisciplinary Plan of Care.
