This position serves as an entry-level designation within the Clinical Advancement Program (CAP) for the Home Therapy Program. As a Home Therapies RN CAP 1, you will be accountable for the provision and coordination of clinically competent care for an assigned group of patients, specifically focusing on home dialysis training and treatment therapy.
You will function as a vital member of the End Stage Kidney Disease (ESKD) healthcare team, participating in decision-making, teaching, leadership functions, and quality improvement activities designed to enhance patient care outcomes. Your day-to-day work involves assessing and managing patients' responses to therapy, collaborating with physicians and the interdisciplinary team to adjust treatment plans, and ensuring the safe and effective delivery of dialysis therapy in the home setting.
A significant portion of your role is dedicated to education. You will assess the learning needs of patients and their families, train Home Dialysis candidates and partners on the operation and maintenance of equipment, and assist in identifying and evaluating candidates for home dialysis options. You will also serve as a resource for the healthcare team, participating in staff training and the orientation of new employees.
This role is primarily based in the home therapy setting, requiring travel to patient homes, training sites, and other facilities. The work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels, though you will also encounter the unique conditions of patient residences. The position may involve exposure to infectious and contagious diseases/materials.
The role requires physical demands including frequent standing, bending, and the ability to move or lift equipment and patients, with assistance as needed. The position may also require participation in an on-call rotation, covering nights, weekends, and holidays to ensure reliable coverage for the program.
