We are seeking a Part-Time Registered Nurse (RN) in Nashua, NH, to deliver essential care within a community setting. In this role, you will utilize the nursing process of assessment, planning, intervention, implementation, and evaluation to support individuals with disabilities. Your work will involve meaningful interaction with individuals, their significant others, and the broader health team, all while upholding the highest standards of professional nursing practice.
You will be an integral part of an interdisciplinary team, developing and updating Individual Support Plans (ISP) and Person Centered Plans (PCP) to reflect the unique needs, observations, and behavioral developments of those you serve. Beyond clinical duties, you will help build a robust support network by fostering relationships with community members, businesses, and families, and by participating in circles of support for personal futures planning.
