The Registered Nurse (RN) Case Manager works within an interdisciplinary team to facilitate the patient plan of care throughout the continuum of care. This role ensures appropriate utilization management, care coordination, resource utilization, and clinical documentation. Functioning within the Mayo Clinical Nursing Professional Practice Model, you will be accountable for assessing, planning, implementing, evaluating, and communicating the patient care plan progression.
You will utilize principles of mutual respect and patient/family advocacy, providing leadership within a team of internal partners and outside agencies. Your goal is to facilitate best practices that achieve quality clinical, financial, and patient satisfaction outcomes. You will bridge communication between providers, patients, families, and the interdisciplinary team to assure high-quality care that meets patient needs and is delivered in a cost-effective and timely manner.
This position offers the opportunity to provide leadership through education on case management concepts, committee work, research, and community involvement. The role is eligible for TN sponsorship.
