This position serves as a vital partner in the Behavioral Med-Clin Psychology department, focusing on the psychosocial well-being of children and their families. The role exists to identify and address complex social situations, safety risks, and barriers to care through advanced interviewing and therapeutic intervention skills.
In this role, you will act as a leader in resource management and interdisciplinary collaboration. You will empower patients and families to be active partners with the medical team, facilitating problem-solving and conflict resolution. Your work involves developing resource networks, advocating for the patient population on a systems level, and educating families on evidence-based treatment options. You will be seen as an expert in providing self-management support to high-risk patients, helping them build the skills and confidence to manage chronic care conditions at home.
You will facilitate family-centered care team meetings and mediate within the medical team on behalf of the patient. By developing programs and protocols, you will help minimize the fragmentation of services and ensure a safe transition of care. This position also offers the opportunity to model and promote behaviors that create a positive work environment while serving as a resource for educating healthcare professionals and students on the psychosocial aspects of care.
