The Patient Care Coordinator serves as a vital link in the continuum of care for our acute inpatient population. This role exists to ensure clinical and cost-effective outcomes by orchestrating a seamless transition from hospital to home or post-acute settings. You will collaborate closely with MAPMG Hospital Based Service Physicians, nursing staff, utilization review teams, and patients and their families to manage the entire care journey.
Your day involves conducting initial face-to-face assessments within 24 hours of admission to identify discharge needs, actively participating in daily Care Without Delay (CWD) rounds, and documenting care progression. You will work to prevent avoidable delays, coordinate with social workers and community providers for long-term care or home health services, and ensure high-risk patients have follow-up appointments scheduled prior to discharge. Success in this position requires navigating a high-volume environment with rapidly changing priorities while maintaining professionalism and empathy.
Candidates must possess the required credentials from the primary work state at the time of hire. Additional credentials for secondary work states will be required within six months of employment. The role demands a Registered Nurse License (Virginia, Maryland, or District of Columbia) or a Compact License, along with Basic Life Support certification. Candidates will also need to demonstrate proficiency with electronic medical record systems and an understanding of health care reimbursement systems.
