You will join the 5 South Cardiovascular team at Seton Medical Center Austin, operating on a full-time night shift schedule. In this role, you will serve as an interdisciplinary leader, coordinating all disciplines to ensure seamless, holistic patient care. Your day will involve implementing customized, evidence-based care plans, monitoring real-time clinical changes, and executing physician directives with precision. You will also manage discharge planning to ensure safe transitions for patients, acting as a vital link between the care team and patient outcomes.
