You will serve as a vital link in the care continuum, implementing evidence-based care plans and coordinating interdisciplinary efforts to ensure seamless, holistic patient care. From monitoring real-time clinical changes to managing discharge strategies, your role is essential in delivering safe and successful patient transitions.
