About the Role

Join our Advanced Heart Failure team at Saint Thomas West in Nashville, TN. As a Registered Nurse on the Night Shift, you will serve as the primary coordinator for all disciplines to ensure well-coordinated patient care. Your day will involve implementing and monitoring comprehensive patient care plans, assessing conditions, and communicating critical updates to the healthcare team. You will carry out physician and nursing orders while actively coordinating discharge planning needs to ensure a smooth transition for patients and their families.
