Senior Community Care of Michigan PACE is part of Volunteers of America National Services (VOANS), serving as the Housing and Healthcare affiliate of the parent organization. Our PACE (Program of All-Inclusive Care for the Elderly) centers bring together clinical professionals, housekeepers, maintenance associates, and culinary employees to provide comprehensive care. About the Role

As a Registered Dietitian, you will play a vital role in the nutritional well-being of our elderly participants. Under the supervision of the Center Manager, you will screen and assess nutritional status, using pertinent data to plan and implement appropriate interventions. Your day will involve coordinating meal services and kitchen facilities to ensure standards are met, while also collaborating with contracted meal services to meet participant needs.

You will function as an integral member of the Interdisciplinary Team, communicating nutritional needs and care plan changes to ensure 24-hour care delivery. This role requires conducting in-person re-assessments annually (or every six months if actively involved in care planning) and monitoring monthly weights, medical diagnoses, drug-nutrient interactions, and hydration status. You will provide nutritional education to participants, caregivers, and other healthcare professionals, documenting all activities in the participant's medical record according to Senior Community Care standards. Additionally, you will provide direction and supervision to Dietary Aides.
