As a Registered Dietitian, you will serve as a vital member of the Interdisciplinary Team, ensuring the nutritional needs of our elderly participants are met. Under the supervision of the Center Manager, you will screen and assess participant nutritional status, using pertinent data to plan and implement appropriate nutrition interventions. Your day will involve conducting in-person assessments, providing nutritional education to participants and caregivers, and coordinating meal services to ensure standards are met.
You will function as a key collaborator in 24-hour care delivery, maintaining regular attendance at team meetings to communicate participant changes and coordinate care planning decisions. Your role includes monitoring monthly weights, medical diagnoses, drug-nutrient interactions, and other health indicators, reporting changes to the team, and making recommendations for adjustments. Additionally, you will provide direction and supervision to Dietary Aides and offer nutritional educational programs to department staff.
