The Clinical Quality Consultant serves as a vital partner in integrating Continuous Quality Improvement (CQI) activities across all sites. This role is designed to support patients and their families in a self-management, self-efficacy, and behavior change model. You will approach your tasks in a team-based care fashion, consulting with sites on the coordination, implementation, support, and monitoring of quality initiatives.
In this position, you will act as a patient advocate for vulnerable populations, ensuring patients are fully informed and comfortable with their visits. You will work in conjunction with medical, dental, and behavioral health providers to ensure the highest standard of integrated healthcare. Your day-to-day involves coaching site quality teams, analyzing workflows for improvement opportunities, and guiding activities based on data analysis to support state and federal quality initiatives.
