We are seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team as a "SNF-ist." In this unique capacity, you will serve as the leading provider of internal medicine services within a Skilled Nursing Facility (SNF) or Assisted Living setting. This role is designed for those who want to be effective by doing the work they love, directly influencing the health outcomes of the geriatric population.
As the clinical leader on-site, you will collaborate closely with the facility's interdisciplinary team, physicians, and nursing staff to oversee the day-to-day care of all residents, including both short-term and long-term patients. You will manage acute and chronic illnesses, perform comprehensive health assessments, and develop patient management policies. Your work will involve conducting physical examinations, prescribing treatments, facilitating referrals, and engaging in advanced care planning. You will also perform monthly stability visits for long-term residents and evaluations for those exhibiting changes in condition.
This position offers a flexible schedule of two nonconsecutive weekdays during daytime hours, allowing you to choose your specific days. The role combines in-person care with virtual charting, supported by company-provided equipment to ensure you have the tools needed to succeed.
