We staff Psychiatric Mental Health Nurse Practitioners (PMHNPs) and General Medicine Nurse Practitioners to act as "SNF-ists"—in-house providers who serve as the leading clinical leaders within Skilled Nursing and Assisted Living facilities. Our goal is to empower you to do the work you love while effectively managing the day-to-day care of residents.

Key Responsibilities
- check_circleAct as the leading provider of internal medicine services for the Skilled Nursing Facility
- check_circlePerform comprehensive physical examinations on short and long-term residents
- check_circleDevelop and implement patient management policies and procedures
- check_circleFacilitate referrals to other healthcare professionals and medical facilities
- check_circleManage advanced care planning and communicate with attending physicians regarding patient care
- check_circleCollaborate with interdisciplinary teams and physicians to oversee resident care
- check_circleMaintain proficient documentation and CPT coding for all patient interactions
- check_circleProvide comprehensive health assessments and differential diagnoses for acute and chronic conditions
- check_circlePrescribe necessary medications and treatments under established protocols
- check_circleConduct monthly stability visits for long-term residents and evaluate patients with changing conditions
Requirements
