We are seeking a compassionate PRN Nurse Practitioner to join our field-based team. In this flexible role, you will serve as a primary point of contact for patients requiring home-based primary and palliative care. Your work will bridge the gap between acute medical needs and long-term wellness, utilizing both in-person visits and telehealth modalities to manage chronic and acute conditions.
You will conduct comprehensive assessments, diagnose conditions, and develop individualized care plans in close collaboration with physicians and care coordinators. Beyond clinical management, a core part of your day involves educating patients and caregivers on disease management, medication safety, and advanced care planning. By working alongside nurses, social workers, and other team members, you will help create a seamless care experience that prioritizes patient dignity and comfort.
