About the Role
As a PRN Nurse Practitioner, you will join a collaborative, mission-driven team focused on reducing avoidable hospitalizations through proactive, in-home management. In this position, you will perform comprehensive in-home assessments, manage complex chronic diseases, and coordinate seamlessly with an interdisciplinary team. You will serve as a vital link between patients, families, caregivers, and community providers, ensuring smooth care transitions and effective self-management education.
