About the Role

As a Nurse Practitioner with HarmonyCares, you will deliver annual risk assessments in residential settings or via telehealth, operating within the scope of practice delegated by a Collaborative Physician. This role is designed for clinicians who thrive in a team environment and are passionate about serving patients in their own homes.

In this position, you will partner with teammates, physicians, medical staff, patients, caregivers, and agency nurses to ensure comprehensive care. You will conduct in-home health risk assessments to identify active and chronic disease conditions, as well as determine physical, mental, and social needs. Your work involves taking patient histories, performing examinations, determining diagnoses, and documenting visits according to NCQA standards. You will also manage case management referrals and communicate with primary care physicians to assure proper diagnosis.
