This role exists to provide unmatched insight into a member's overall health and well-being that is often difficult to capture during routine office visits. During a visit lasting up to one hour, you will review and observe a member's current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) allows you to improve quality of care and potentially close multiple care gaps in a single visit.
You will be the face of personalized care, conducting adult and geriatric assessments that include medical history, diagnosis, treatment, and health education. Your day involves collaborating with Primary Care Physicians, case managers, social workers, family members, and caregivers to ensure seamless follow-up and clear documentation. Whether visiting members in their homes or Skilled Nursing Facilities, you will administer point-of-care and lab-based testing as needed, leveraging your expertise to make a direct impact on the health and well-being of others.
