As a Home Health Occupational Therapist Assistant, you will report to the Clinical Manager and provide therapy services along with vocational and education programs delegated by the qualified Occupational Therapist. You will train patients and family members to manage retraining programs, including dressing, feeding, grooming, and hygiene skills, while documenting observed information and progress reports in patient records. Additionally, you will participate in care coordination and discharge planning activities, acting as a resource to other healthcare personnel in meeting patient needs.
