As a Home Health Occupational Therapist at CenterWell Home Health, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
In this position, you will assess and screen patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives. You will administer diagnostic tests to assist physicians in evaluating functional levels and collaborate with the patient's physician and the broader healthcare team to develop and revise Plans of Care. Your day-to-day involves providing therapeutic treatment to improve strength, coordination, and range of motion, while also consulting with vocational team members to implement vocationally oriented plans.
You will be a key resource in care coordination and discharge planning, designing community reintegration activities to assist clients in physical reconditioning and psychological adjustment. Additionally, you will recommend adaptive equipment to improve living or working environments and train staff and family members on retraining programs for daily skills like dressing and feeding. This role also includes supervising and evaluating Occupational Therapy Assistants and Home Health Aides, as well as accurately documenting all patient care observations and interventions.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. As this position involves driving to patient locations, it is part of Humana's driver safety program, requiring a valid state driver's license and personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
