About the Role

As a Home Health Occupational Therapist at CenterWell, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.

In this position, you will partner with physicians and the interdisciplinary team to develop and revise Plans of Care. Your day-to-day involves assessing patients' daily living and work-related skills, administering diagnostic tests, and providing therapeutic treatment to restore function or teach compensation measures. You will also design community reintegration activities, recommend adaptive equipment, and train staff and family members to carry out retraining programs. Additionally, you will supervise Occupational Therapy Assistants and Home Health Aides, ensuring high-quality service delivery and accurate documentation of all patient care observations and interventions.

Hiring Process & Application

This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. As this position involves driving to patient homes, it is also part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
