As a Registered Nurse, you will oversee the complete operation and performance of dialysis treatment and care delivery within a Skilled Nursing Facility. Operating under the contract between Rendevor Dialysis and the facility, you will serve as a vital resource to staff, physicians, and physician assistants, utilizing your professional nursing knowledge and critical thinking skills to ensure compliance with state and federal regulations.
Your day-to-day involves conducting comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs to identify gaps in care. You will collaborate with the patient care team to create and implement care plans that address these needs and improve patient health. Acting as an advocate for patients and their families, you will facilitate continuity of care across various settings, including home, hospital, and acute care facilities, while managing transitions to minimize avoidable readmissions. Additionally, you will provide education and self-management support, monitor quality control measures, and participate in Quality Assurance and Performance Improvement (QAPI) committees.
This role is situated in the medical unit of a post-acute facility. The position may involve exposure to chemicals and blood-borne pathogens and requires the routine use of both medical and standard office equipment. The schedule may range from five to fifteen hours per day based on unit requirements, such as patient census and staffing needs.
