This position serves as a vital resource for staff, physicians, and physician assistants, utilizing professional nursing knowledge and critical thinking skills to oversee the complete operation and performance of dialysis treatment delivery. You will ensure care is provided under the terms of the contract between Rendevor Dialysis and the Skilled Nursing Facility, strictly adhering to state and federal regulations.
Your day-to-day involves providing clinical oversight to non-licensed support teams, training facility staff, and conducting comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs. You will collaborate with the patient care team to create and implement care plans that address identified needs and remove barriers to health. Additionally, you will coordinate care across various settings, manage transitions to minimize readmissions, and facilitate continuity of care spanning from home to acute care facilities.
This role is situated in the medical unit of a post-acute facility. The position may involve exposure to chemicals and blood-borne pathogens and routinely requires the use of both medical and standard office equipment. The schedule may range from five to fifteen hours per day based on unit requirements, such as patient census and staffing needs.
