About the Role

This position offers a unique opportunity to serve Veterans within the VA's Patient-Aligned Care Team (PACT) model. As a primary care physician, you will practice "Whole Health," an approach that empowers patients to take charge of their well-being while living their lives to the fullest. You will work in a collaborative environment where your dedicated team includes a Registered Nurse Care Coordinator, Clinical Associate, Administrative Associate, Clinical Pharmacy Practitioner, Social Worker, and Mental Health staff. This model ensures that every Veteran receives personalized, patient-centered care that addresses their physical, functional, and psychosocial needs.

In this outpatient clinic setting, you will manage an assigned panel of patients, seeing approximately 12-14 patients per day. Your day will involve providing excellent clinical judgment and medical knowledge, from medical interviewing and physical examinations to procedural skills. You will initiate and facilitate treatment in emergency situations, coordinate care with community healthcare entities, and utilize Tele-Health tools like VA Video Connect for home visits. The role emphasizes a humanistic approach, ensuring that care plans align with each Veteran's specific goals and diagnoses.

About the Location

You will be based in Cambridge, Maryland, a historic city and county seat on the Eastern Shore located on the Choptank River near the Chesapeake Bay. Cambridge is a compact, walkable community with a distinct maritime atmosphere, featuring waterfront parks and the Richardson Maritime Museum, which showcases 300 years of local boatbuilding heritage. Just 12 miles south lies the Blackwater National Wildlife Refuge, placing you in the heart of the Chesapeake Bay's rich ecological and cultural landscape. With a population of about 13,000, Cambridge blends historic charm with modern amenities, offering a strong sense of community through boating, tourism, and local events. Its motto, "Living, Working, Relaxing...And Loving It," reflects the welcoming character of the area.
