As a Primary Care Physician, you will manage a dedicated patient panel of adults on Medicare, focusing on prevention, chronic disease management, and acute care. This role exists to leverage data and innovative care models to reduce hospitalizations and close care gaps. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to deliver coordinated, high-quality care.
In this position, you will not only provide clinical assessments and develop individualized treatment plans but also engage patients and families in shared decision-making.
