As a Primary Care Physician, you will manage a dedicated panel of adult and Medicare patients, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes for a population that often faces significant health disparities. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to deliver coordinated, evidence-based care.
In your day-to-day, you will conduct detailed history and physical examinations, develop individualized treatment plans, and engage patients and families in shared decision-making. You will leverage population health data to drive quality improvement initiatives and participate in regular team huddles and case reviews. Beyond clinical duties, you will contribute to a positive clinical culture by upholding our values of inclusivity and collaboration while pursuing ongoing professional development.
