As a Primary Care Physician, you will manage a dedicated panel of adult Medicare patients in an outpatient clinic setting. This role exists to bridge the gap in care for diverse, underserved populations by focusing on prevention, chronic disease management, and holistic well-being. You will collaborate closely with an interdisciplinary team—including nurses, care managers, behavioral health clinicians, and social workers—to deliver coordinated, evidence-based care.
Your day-to-day involves conducting thorough history and physical exams, developing individualized care plans for patients with multiple chronic conditions, and utilizing population health tools to close care gaps. A key component of this position is the ability to communicate fluently in Spanish, ensuring that diagnoses, treatments, and care plans are clearly understood by patients and families. You will also participate in daily team huddles, quality improvement initiatives, and on-call coverage to support clinic operations.
