About the Role
In this position, you will manage a dedicated panel of adult Medicare patients in an outpatient clinic setting. The role focuses on prevention, chronic disease management, and acute care, requiring you to develop and implement evidence-based, individualized care plans for patients with complex medical and social needs.
You will collaborate closely with nurses, medical assistants, behavioral health providers, care managers, and specialists to coordinate care and support transitions from hospitals and post-acute facilities. A key component of this role is the ability to communicate fluently in Spanish with patients and families to explain diagnoses, treatments, and care plans effectively. You will also participate in daily team huddles, case reviews, and quality improvement activities to drive clinical quality and value-based care performance targets.
