As a Primary Care Physician, you will deliver preventative, evidence-based primary care and manage chronic conditions for a defined patient panel. This role is designed to build longitudinal relationships with patients, supporting their care planning and engaging in quality improvement initiatives. You will work within an interdisciplinary team—including nurses, medical assistants, care managers, and behavioral health staff—to enhance patient outcomes and ensure continuity of care.
Your day-to-day involves conducting thorough primary care visits, developing individualized care plans for patients with complex chronic conditions, and coordinating with specialists and hospitals to prevent readmissions. We emphasize culturally responsive care, with a strong preference for Spanish language skills to better serve our diverse patient population.
