As a Primary Care Physician, you will manage a dedicated panel of adult Medicare patients in an outpatient clinic setting. This role exists to bridge the gap in care for vulnerable populations, focusing heavily on prevention, chronic disease management, and acute care. You will collaborate closely with an interdisciplinary team—including nurses, medical assistants, behavioral health providers, and care managers—to deliver holistic care.
A key component of this position is the ability to communicate fluently in Spanish with patients and their families, ensuring clear explanations of diagnoses, treatments, and care plans. You will be responsible for developing evidence-based, individualized care plans for patients with complex medical and social needs, coordinating care transitions from hospitals, and utilizing the electronic health record to track outcomes and close care gaps.
