As a Primary Care Physician, you will manage a dedicated panel of adult and Medicare patients, focusing on prevention, chronic disease management, and acute care. This role exists to bridge the gap in care for vulnerable populations by leveraging data and innovative care models to reduce hospitalizations and close care gaps. You will work within an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to deliver coordinated, evidence-based care.
Your day will involve conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. You will utilize population health data to drive quality improvement initiatives and ensure continuity of care through collaboration with specialists and community resources. Beyond clinical duties, you will participate in regular team huddles and professional development opportunities, contributing to a positive clinical culture that values patient-centered, inclusive, and collaborative care.
