As a Primary Care Physician, you will manage a dedicated patient panel of adults on Medicare, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes for a vulnerable population by leveraging evidence-based guidelines and population health data. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to coordinate seamless care and ensure continuity with specialists and community resources.
Your day-to-day involves conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. Beyond clinical duties, you will participate in team huddles, case reviews, and quality improvement initiatives, all while upholding our values of patient-centered, inclusive, and collaborative care.
