About the Role
As a Primary Care Physician, you will manage a dedicated patient panel of adults on Medicare, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes through innovative care models and data-driven insights. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to deliver coordinated, evidence-based care.
Your day will involve conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. You will leverage population health data to reduce avoidable emergency visits while ensuring compliance with regulatory and payer requirements. Beyond clinical duties, you will participate in quality improvement initiatives and team huddles, contributing to a positive clinical culture that values patient-centered, inclusive, and collaborative care.
