About the Role

As a Primary Care Physician, you will manage a dedicated patient panel, focusing on prevention, chronic disease management, and acute care visits for an adult and Medicare population. This role exists to close care gaps and improve outcomes through evidence-based guidelines and population health data.

Your day will involve conducting detailed history and physical examinations, ordering and interpreting diagnostic tests, and developing individualized treatment plans for common chronic conditions such as diabetes, hypertension, COPD, and heart failure. You will coordinate care with a multidisciplinary team—including nurses, medical assistants, behavioral health specialists, and community health workers—to ensure continuity and coordination of care.

Beyond clinical duties, you will engage patients, families, and caregivers in shared decision-making and education to support adherence to treatment plans. You will also participate in regular team huddles, case reviews, and quality improvement initiatives while maintaining up-to-date clinical knowledge through ongoing professional development.
