As a Primary Care Physician, you will manage a dedicated panel of adult and Medicare patients, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes by leveraging data and evidence-based guidelines. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to coordinate seamless care and ensure continuity with specialists and community resources.
Your day-to-day involves conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. You will utilize population health data to drive quality improvement initiatives and participate in regular team huddles and case reviews. Beyond clinical duties, you will contribute to a positive clinical culture that upholds our values of inclusivity and collaboration, while enjoying opportunities for mentorship and leadership development.
