About the Role

The Primary Care Physician (PCP) at ChenMed is more than a consultant for sickness; you are a health coach and a leader. You will demonstrate accountability for outcomes for a panel of up to 450 patients, building trusting relationships that help patients change behaviors and achieve their health goals.

In this role, you will simplify and prioritize care to make behavior change actionable for both patients and your care teams. You will work closely with managers and medical directors to ensure compliance with guidelines while participating in risk and quality management programs. As a leader within your center, you are expected to champion a positive culture of love, accountability, and passion alongside your center leadership.

You will function both independently and collaboratively, leading a care team that includes care promoters, facilitators, and coordinators. Whether managing acute and chronic illnesses in the office or engaging with hospitalists and transitional care teams for homebound patients, your focus remains on high-quality, evidence-based care that considers lifestyle and nutrition.
