As a Primary Care Physician, you will manage a dedicated panel of adult and Medicare patients, focusing on prevention, chronic disease management, and acute care. This role exists to close care gaps and enhance outcomes for vulnerable populations by leveraging evidence-based guidelines and population health data. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to coordinate seamless care and ensure continuity with specialists and community resources.
Your day-to-day involves conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. You will lead quality improvement initiatives, participate in team huddles, and utilize the electronic health record to ensure accurate documentation and regulatory compliance. Beyond clinical duties, you will contribute to a positive clinical culture that upholds our values of inclusivity and collaboration.
