As a Primary Care Physician, you will manage a dedicated patient panel, focusing on prevention, chronic disease management, and acute care. This role exists to leverage data and innovative care models to reduce hospitalizations and close care gaps for older adults. You will collaborate closely with an interdisciplinary team—including nurses, behavioral health specialists, and community health workers—to ensure continuity of care and enhance patient outcomes.
Your day will involve conducting detailed history and physical examinations, developing individualized treatment plans, and engaging patients and families in shared decision-making. By utilizing evidence-based guidelines and population health data, you will drive quality improvement initiatives that make a meaningful impact on community health every day.
